Hyaluronic acid (HA) fillers and autologous micro fat grafting are the two credible ways to add lasting volume and shape to the lips. They are not competitors — they answer different questions. Fillers are about quick, reversible, predictable enhancement. Fat grafting is about structural, long-term restoration using your own tissue. This guide explains which is right for you.
Quick verdict
- Choose HA fillers if you are new to lip enhancement, want a result you can adjust or dissolve, and accept a 6–12 month maintenance cycle.
- Choose micro fat grafting if you want a one-time, structural, lasting enhancement using your own tissue, are willing to accept a short downtime, and ideally are already having another procedure under sedation.
Side-by-side comparison
| Feature | HA Lip Fillers | Micro Fat Grafting |
|---|---|---|
| Material | Cross-linked hyaluronic acid gel | Your own purified micro-fat |
| Setting | In-clinic, awake | Operating room, sedation or light general |
| Anesthesia | Topical / dental block | Sedation + local |
| Session length | 30–45 min | 60–90 min (lips only); longer if combined |
| Downtime | 1–3 days swelling | 5–10 days swelling and bruising |
| Longevity | 6–12 months | Permanent for surviving graft (typically 50–70% take) |
| Reversible | Yes — hyaluronidase | No (revision is possible but surgical) |
| Texture | Soft, slightly hydrophilic | Indistinguishable from native lip tissue |
| Top-up needed | Yes, every 6–12 months | Sometimes a single touch-up at 6 months |
| Best for | First-time enhancement, adjustability, perioral lines | Structural restoration, deflated thin lips, patients tired of maintenance |
| Avoid if | You want a one-time permanent change | You have very low body fat or want guaranteed reversibility |
How HA lip fillers work
A pre-formulated hyaluronic acid gel is injected with a fine needle or blunt cannula into specific layers of the lip — submucosal for volume, vermilion border for definition, wet-dry junction for eversion. Results are visible immediately, settle by 4 weeks, and gradually dissolve over 6–12 months. The big advantages are predictability and reversibility: if you do not like it, hyaluronidase dissolves the product in 24–48 hours. The full picture is in our guide to HA lip fillers.
How autologous micro fat grafting to the lips works
Micro fat grafting uses your own fat as a permanent filler. The process:
- Harvest — fat is gently aspirated from a donor site (inner thigh, abdomen, or flank) using fine, low-pressure cannulas to protect adipocyte viability
- Processing — the lipoaspirate is centrifuged or filtered, and emulsified into micro-fat or nano-fat particles small enough to pass through fine cannulas without trauma
- Injection — micro-droplets are placed in multiple passes and multiple planes of the lip, so each graft particle sits next to a blood supply and has the best chance of survival
The grafted fat that "takes" (typically 50–70%) becomes a permanent part of your lip — vascularized, soft, and indistinguishable from native tissue. The fat that does not survive is reabsorbed in the first 3 months, which is why we slightly over-correct.
Results and longevity
HA fillers give you a result you can see the moment you leave the clinic, but it is on a maintenance cycle. Over 5 years you will typically spend more on fillers than on a single fat grafting session.
Fat grafting looks under-corrected for the first 1–2 weeks (swelling masks the loss), then a real picture emerges at 3 months. What is there at 6 months is essentially permanent. The texture is the closest thing to "your lips, just better" that aesthetic medicine offers.
Risks and recovery
| Aspect | HA Fillers | Fat Grafting |
|---|---|---|
| Bruising | Common, mild, 3–7 days | Common, moderate, 7–14 days |
| Swelling | Moderate, peaks at 24 h | Significant, peaks at 48–72 h |
| Vascular event | Rare but serious; reversible with hyaluronidase | Very rare with micro-droplet technique |
| Lumps / irregularity | Usually technique-related, treatable | Possible if over-grafted; revisable |
| Donor-site morbidity | None | Mild bruising at harvest site, no contour issue |
| Time off social activity | 1–2 days | 5–10 days |
Who is the ideal candidate for each?
HA fillers are ideal when:
- You have never had lip enhancement and want to "try" the look
- You want a result you can dissolve if needed
- Your primary concern is the vermilion border, Cupid's bow definition, or vertical lip lines
- You cannot take 5–10 days of social downtime
Fat grafting is ideal when:
- You have committed to lip enhancement and want a one-time, lasting result
- You have thin or age-deflated lips and want a soft, natural-textured restoration
- You are already booked for another facial procedure (facelift, rhinoplasty, blepharoplasty) and can add lips under the same sedation
- You are tired of the cost and rhythm of repeat filler sessions
Can you combine them?
Yes, and it is one of our most common protocols. Fat grafting builds the foundation; small touch-ups of HA filler later refine the vermilion border or correct minor asymmetries. The order matters: fat first, filler later. Injecting HA into a recently grafted lip can compromise graft survival.
