Toe-to-Thumb Transfer: Microsurgery, Teamwork and Mentorship

Tıbbiye Bülteni recently reported a rare reconstruction at the Plastic, Reconstructive and Aesthetic Surgery Clinic of Istanbul Okmeydanı Prof. Dr. Cemil Taşçıoğlu City Hospital. A 16-year-old lost all the fingers of his right hand in a meat-grinder accident. Replantation was not possible, so the second toe of his left foot was transferred to the hand as a thumb, in an operation of about ten hours.

Why the thumb matters

The thumb accounts for close to half of hand function, because every grip depends on the fingers meeting it. Without it, holding, writing or eating become very difficult.

What a toe-to-hand transfer involves

A toe is the closest anatomical match to a finger. It is moved together with its bone, tendons, artery, veins and nerves, which are reconnected under the operating microscope. The goal in this case was to restore a basic power grasp and, with it, independence.

The microsurgical challenge

• Tiny structures: vessels and nerves around a millimetre or less in diameter must be joined precisely.
• Ischaemia time: the period without blood flow must be kept short to protect the tissue.
• Bone and tendon balance: correct alignment and tension let the new thumb move.
• Endurance: the surgical and anaesthesia teams must stay focused for many hours.

A team effort, and the role of mentorship

As reported, the operation was carried out by Op. Dr. Mehmet Fatih Çamlı, the residents and the wider hospital team, with the support of Prof. Dr. Mehmet Veli Karaaltın. Sharing reconstructive experience, from facial reanimation to composite tissue transplantation, helps younger surgeons in public hospitals take on complex cases, and each such case becomes a reference for future patients.
