Correcting Tubular Breasts Without Implants: Fat Grafting and the Twirl Internal Bra

Many patients with tubular breast deformity do not want an implant. They want the breast to look normal — a full lower pole, a correct fold, a flat areola — without carrying a device for life. In grade I and most grade II deformities that goal is achievable using the patient''s own tissue, provided the operation does three things in the right order: release, redistribute, and support.

Step 1 — Releasing the constriction

The constricting fibrous ring at the breast base is divided from a periareolar or inframammary access. Radial scoring of the deep glandular surface allows the compressed parenchyma to unfurl. Without this step, any volume added simply projects forward through the areola and reproduces the deformity in a larger form.

At the same time the inframammary fold is taken down and re-set at its anatomically correct level, and the lower pole skin envelope is expanded to accept tissue that has never occupied it.

Step 2 — Structural fat grafting

Fat is harvested atraumatically, processed to a consistent, cell-rich graft, and placed in multiple fine planes — subcutaneous, retroglandular and pre-pectoral — rather than as a single bolus. Placement priorities in a tubular breast are specific:

    • Lower pole, to create convexity and mask the released fold line.
    • Medial base, which in grade I deformity is the primary deficiency.
    • Upper transition, only lightly — over-filling the upper pole is what makes a corrected tubular breast look operated.
    • Thin skin zones, where grafting adds dermal thickness and improves the quality of the envelope for the future.

Fat grafting is also the most precise instrument available for asymmetry. Tubular deformity is rarely symmetrical, and differences of 30–60 cc between sides are corrected far more accurately with graft than with implant sizing.

Step 3 — The Twirl Internal Bra

Release and grafting solve today''s shape. The Twirl Internal Bra protects it. The technique uses a superolateral pedicle, which preserves a robust and reliable blood supply to the nipple-areola complex, and rotates and secures the parenchyma as an internal sling — an autologous bra formed from the patient''s own gland rather than mesh or scaffold alone.

In tubular breasts this matters for two reasons. First, released parenchyma has a natural tendency to re-herniate through the areola; the internal sling resists that. Second, thin lower-pole skin cannot hold new volume by itself, so the load must be carried internally, not by the skin envelope.

Preserving areolar sensation is a design goal of the superolateral pedicle, not an incidental benefit — the innervation of the nipple-areola complex runs with the pedicle rather than being crossed by it.

Who is a good candidate for the implant-free route

    • Grade I or grade II deformity.
    • Adequate donor fat — typically abdomen, flanks or thighs.
    • A realistic volume goal: expect one to one-and-a-half cup sizes per session.
    • Stable weight, non-smoker, willing to accept that a second refinement session may be advisable.

    Who is not

Patients with very little donor fat, grade III deformity with a severely narrow base and minimal gland, or those who want substantial upper-pole projection are better served by a hybrid approach combining a modest implant with grafting and the Twirl support.

    Recovery

Most patients return to desk work within a week. Compression of the donor site is worn for four weeks, no direct pressure on the breasts for three, and full sport at six. Graft retention is judged at three months, and final shape at nine to twelve.

    Frequently asked questions

How much of the transferred fat survives?

Typically 55–75% with careful harvesting and multi-plane placement. Released, well-vascularised recipient tissue retains graft better than a compressed constricted breast, which is one more reason release must come first.

Does fat grafting interfere with mammography?

Post-grafting oil cysts and calcifications have recognised, benign radiological appearances. Always tell your radiologist that you have had fat grafting.

Will the areola be reduced in the same operation?

Yes. Areolar reduction and flattening of the herniation are part of the same procedure in almost every tubular case.

Is the Twirl Internal Bra permanent?

The support is created from the patient''s own tissue and is permanent, though it does not stop the normal effects of ageing, pregnancy or large weight changes.
