Lipoedema or Weight Gain? Stages, Symptoms and Finding a Lipoedema Specialist

Prof. Dr. Mehmet Veli Karaaltın September 7, 2026 5 min read

Most women I meet with lipoedema have spent years being told to lose weight. They did lose weight — from the face, the waist, the arms — while the legs stayed exactly the same, and stayed painful. That pattern is the clue.

Signs that point to lipoedema rather than weight gain

    • Symmetry. Both legs are affected almost identically. Ordinary weight gain is more diffuse.
    • Disproportion. A markedly heavier lower body with a comparatively slim upper body and waist.
    • The ankle cuff sign. The fat stops abruptly at the ankles, leaving the feet unaffected — unlike lymphoedema, which involves the feet.
    • Pain and tenderness. Legs ache, feel heavy by evening, and hurt when pressed. This is the defining symptom.
    • Easy bruising. Bruises appear without remembered injury, from fragile small vessels in the affected tissue.
    • Resistance to diet and exercise. The affected fat does not respond proportionally to weight loss.
    • Hormonal timing. Onset or worsening around puberty, pregnancy or menopause.

    Why the distinction matters

If lipoedema is treated as obesity, the plan is diet, and the plan fails. If it is recognised as lipoedema, the plan is symptom control — compression, manual lymphatic drainage, movement, weight stability — and, where pain persists, surgery. The 2026 LIPLEG randomised trial in The Lancet showed that 68% of women achieved meaningful leg pain reduction after liposuction, against 8% with conservative therapy alone. Getting the diagnosis right is what unlocks that option.

    Staging, in plain terms

Stage I: skin still smooth, fat tissue thickened underneath. Stage II: uneven, nodular skin surface with dimpling. Stage III: large overhanging tissue lobes distorting the leg contour and limiting movement. Pain does not always track with stage — some stage I patients hurt a great deal.

What to bring to a consultation

Photographs of your legs over the years, a note of when symptoms started, your weight history, any compression therapy you have tried and for how long, and a simple daily pain score. That history usually tells more than any single examination.

Getting a diagnosis in the UK

Lipoedema is diagnosed clinically, not by scan. In the UK the usual route is a GP referral to a vascular, lymphoedema or plastic surgery clinic; waits are long and many women are told to lose weight first. Bring photographs over time, a note of family history and a record of how the legs respond to dieting — that documentation matters later if you apply for funding or seek surgery.

Getting a diagnosis in the United States

US patients usually reach a diagnosis faster through a lymphatic or plastic surgery specialist, and a written diagnosis with staging is what insurers ask for before considering coverage of treatment. Conservative therapy — compression, manual lymphatic drainage, exercise — is normally required and documented first.

Not sure which one you have?

A video consultation can usually tell within minutes. Contact the clinic, or read what the Lancet LIPLEG trial found about surgery.

Lipoedema stages 1 to 4: what each one looks like

Stage 1: skin still smooth, fat is soft and increased, legs already ache and bruise easily. Stage 2: the skin surface becomes uneven and dimpled, with nodules you can feel under the fingertips. Stage 3: large overhanging lobules form at the thighs and knees and walking changes. Stage 4: lipoedema together with lymphoedema (lipo-lymphoedema), with swelling that no longer settles overnight. Staging matters because it shapes how many operating sessions are realistic, not whether you can be treated.

Searching for a lipoedema specialist near you

Whether you type lipoedema specialist in the UK or lipedema doctor near me in the US, judge the clinic on the same four points: do they stage the disease and photograph it, do they use a lymph-sparing technique rather than aggressive suction, do they plan treatment in sessions by body region, and do they give you a written compression and follow-up protocol. A surgeon who answers all four is worth travelling for; one who quotes a single price for "legs" without an examination is not.

    Insurance and funding, briefly

In the UK, lipoedema surgery is not routinely commissioned and most patients go through an Individual Funding Request, which is frequently refused. In the US, cover for lipedema reduction surgery varies by insurer and usually requires documented conservative therapy, photographs and a staging diagnosis. Keep every clinic letter, compression prescription and therapy record — those documents are what the decision is made on.
