## Why breast surgery changed
For decades, a breast lift depended on the strength of the patient''s own tissue to hold the result. When that tissue was thin or stretched by pregnancy, weight change or an ageing implant, recurrence was a matter of time. The last few years have shifted the problem from *shaping* to *supporting*.
## The internal bra concept
An "internal bra" is not a product; it is a principle. A supporting scaffold is placed inside the breast to carry the weight of tissue or implant while the body deposits its own collagen along it.
**GalaFLEX** is the best-known scaffold: a resorbable poly-4-hydroxybutyrate mesh that holds substantial load early, then is gradually replaced by organised collagen. It is used to support the lower pole in a lift, to reinforce a weak inframammary fold, and to stop recurrent bottoming-out in revision cases.
The candidate is usually the patient with thin skin, a heavy breast, a previous lift that dropped, or an implant exchange where the pocket has stretched.
## Regenerative adjuncts
**BlooCell** and comparable fat-processing systems concentrate the regenerative fraction of harvested fat so grafted volume survives better and soft tissue quality improves. In aesthetic breast work the use is refinement rather than volume: softening implant edges, correcting upper-pole hollowing, and improving skin quality over thin coverage. It supplements a lift or exchange; it does not replace one.
## Implant exchange: the operation most patients underestimate
Exchange is rarely a simple swap. The decisive questions are the state of the capsule, whether the pocket has stretched, whether the fold has descended, and whether the nipple position still matches the new implant. Doing only the swap is exactly why some patients are disappointed twice.
A modern exchange plan typically combines: capsule management, pocket control or fold reinforcement (often with mesh), a lift where the nipple sits low, and fat grafting for contour.
## AI-assisted implant selection
Implant choice used to be a conversation about cup size. It is now a measurement problem: base width, skin stretch, notch-to-nipple distance, tissue thickness and chest asymmetry.
AI-assisted planning software takes 3D surface scans and measurements and narrows thousands of implant permutations to the small set compatible with your anatomy, then simulates the projected result. Its value is realism — it removes sizes that will not fit your chest, and it makes the conversation objective. It does not replace surgical judgement, and no simulation is a guarantee.
## What to ask
- Does my tissue need support, or only reshaping?
- If mesh is proposed, why — and what happens if we do not use it?
- For an exchange: what is being done about the pocket and the fold, not just the implant?
- What implant dimensions suit my chest measurements, and why?
