## The single distinction that matters
Almost every modern facelift claims to lift "deep tissue". The honest test is whether the retaining ligaments are released.
A **SMAS facelift** tightens the fibrous sheet lying over the deep facial tissues — by folding it (plication) or cutting and advancing it. It improves the jawline reliably. It does not move the midface much, because the zygomatic and masseteric ligaments still anchor the cheek where it sits.
A **deep plane facelift** enters *beneath* that sheet and divides those ligaments. Cheek, jawline and neck are then repositioned upward as one composite unit, and skin is redraped without tension. That is why the result reads as rested rather than pulled, and why the structural benefit tends to last longer.
A **mini lift** is a shorter-scar skin and SMAS procedure. A **thread lift** suspends tissue temporarily and releases nothing. Neither addresses true midface descent.
## What this means for your result
| | SMAS | Deep plane |
|---|---|---|
| Midface / cheek lift | Limited | Direct |
| Jawline and neck | Good | Good, with platysma repair |
| Skin tension on closure | Higher | Minimal |
| Typical structural longevity | Shorter | Ten years or more |
| Technical demand | Moderate | High — anatomy-dependent |
The deep plane is not automatically the right operation for everyone. Where the concern is skin quality rather than descent, a lift is the wrong answer and should be declined.
## Cost: London versus Istanbul, compared honestly
A private deep plane facelift in London or on Harley Street sits at the very top of UK aesthetic pricing, and the figure quoted is usually the **surgeon's fee alone**. Hospital, anaesthetist, garments and follow-up appear separately.
Before comparing any Istanbul figure, ask your UK clinic for one total including theatre, anaesthetist, overnight stay and all follow-ups. That is the only like-for-like number. Istanbul packages are quoted as a single all-inclusive figure; the difference comes from staffing, hospital and insurance costs in Turkey — not from a different standard of accreditation, anaesthesia or implants.
## Four questions to ask any surgeon
1. How many true deep plane lifts do you perform each year?
2. Do you release the zygomatic and masseteric retaining ligaments, or plicate the SMAS?
3. Who performs my operation on the day, from first incision to closure?
4. What exactly does the quoted figure include — and exclude?
A surgeon who answers all four plainly is the surgeon to consider, wherever they practise.
## Recovery for patients travelling from the UK
Plan 8–10 nights in Istanbul. Sutures and dressings are managed in person and you are cleared to fly only after a final check. Home-based desk work is realistic around two weeks, office and social life at three to four weeks, exercise from four to six weeks. Residual swelling settles over about three months.
